Developmental Special Instruction
We began Early Intervention services in 2001 working with families and their children in their homes. Developmental Special Instruction, within the early intervention setting, is a service provided to children age birth to three. Developmental Special Instructors work with children in their natural environment, usually the home or community. They work with the child and family to increase developmental skills within six domains: gross and fine motor, language, social, self-help, and cognitive skills. Documentation is maintained to track progress toward Individualized Family Service Plan (IFSP) outcomes, and to verify services delivered. On their visits, Developmental Special Instructors (DSI) will provide intervention and treatment designed to implement the IFSP outcomes.
Therapy services
We offer Speech, Occupational, and Physical therapies. Each child is evaluated and assessed on individual need. The therapist will work closely with the family and other team members while providing treatment. All therapies will take place in the child’s home or community. Parents are encouraged to participate in home visits and learn how to support each child’s potential in all areas of development. The team will work together with you and your child to develop fine and gross motor skills, communication, and sensory integration. We will also provide home activities and exercises for you and your child to foster emerging skills. The goal is to help your child reach their highest level of independence in everyday life.
Home and Community Based Services
Family Partners provides habilitation, respite, and attendant care for children and adults in their home or community. Respite services will provide short-term relief for the primary caregiver to either go and run some errands or just have time for them-self. These services can be provided any time of the day or evening that the caregiver requests. Habilitation services are for individuals that need assistance with working on outcomes that are described in an Individual Service Plan. The certified habilitation worker will come and help the individual meet their outcomes in the home or community. Attendant care is to help an individual with their personal hygiene, assist in dressing, and helping with meals. Attendant care can be received in the home or community.
Day Treatment and Training Center for Adults
This program for adults offers training in daily living skills to help each person reach their potential in their daily life. Some of the skills we will be teaching are: cooking, personal hygiene, fitness, computer skills, social skills, and many more skills. We will also facilitate many trips in the community. This is the best place to be during the day, learning while having a great time! Family Partners provides a safe and nurturing environment where each person served will have an Individualized Support Plan (ISP) to assist them in reaching their goals, and full potential. Developmental Home Program
Growing one family at a time! Family Partners LLC is an organization dedicated to assisting, educating, and sponsoring families who have chosen to share their home and family environment to improve the life of an individual with disabilities. Families and single applicants may choose to become licensed as either a Child or Adult Developmental Home. Your Family Partners Licensing Worker will help guide you through the entire state mandated licensing process from start to finish and will be accessible and helpful at all times. Your licensing packet will be submitted in a timely manner and once the license is received, we will actively pursue a compatible placement for you to include in your family. Once a placement is made, your Licensing Worker will continue to offer support and make sure everything is running smoothly. We value our families and treat them with respect. Our mission is to provide services that are person-centered, family-friendly, uncomplicated, accessible, and quality driven.
